Don’t Get Lost in the System: Navigating Hospitalizations and Care Transitions

Learn how to advocate for your aging loved one during hospital stays and after discharge. This program offers practical tips for communicating with healthcare providers, managing medications and health records, scheduling follow-up care, and preparing the home for recovery. We’ll also review common post-discharge care options—such as home health, rehabilitation, and skilled nursing—to help families choose the right level of support and reduce the risk of avoidable re-hospitalizations.

Who We Are

Welcome to ACAP Decatur-DeKalb, whose core mission is to provide information, resources, support, and a sense of community for adult children who are caring for their aging parents, while also encouraging these caregivers to prioritize their own well-being. Our chapter is explicitly designed to assist families in caring for older adult loved ones and to provide essential support for caregivers.

A core component of ACAP Decatur-DeKalb's offerings is its educational programs, which are based on a nationally validated curriculum. These programs are delivered through various channels, including monthly programs, audio podcasts, videos, annual conference, and "Lunch & Learns" such as the "First Fridays with ACAP" series.

Each month, ACAP Decatur-DeKalb will host a FREE educational program that will provide subject matter experts on subjects related to the aging process and caregiving. We cover topics like local healthcare resources, legal matters for seniors, and financial planning for you and your aging parents. These gatherings will also provide an opportunity for fellowship and to build camaraderie among the attendees.

We invite you to join us for free monthly ACAP programs that address topics common to most caregivers for aging adults. We will provide light refreshments and an opportunity to learn about local resources and experts who serve the senior population of DeKalb County.
